POSITION SUMMARY:
This position is responsible for improving the quality of care that patients receive through organization-wide quality initiatives and improvement activities.
DUTIES AND RESPONSIBILITIES:
Assists in the planning, training, implementing, monitoring and reporting out of quality improvement activities.
Assists in the planning, implementing and monitoring of new care initiatives for medical, dental, and behavioral health.
Collaborates with clinical staff and providers to provide comprehensive, team-based care to all patients.
Performs patient chart audits as part of a QI project, payer request, or regulatory requirement.
Assists support staff and providers with improving workflows and documentation to meet clinical standards of care and close care gaps.
Educates support staff and providers on clinical quality measures.
Assists with audit findings including preparation and implementation of QI activities to address deficiencies.
Understands HEDIS measures and various health plan requirements, including Medicaid and Medicare plans.
Assists with payer requests for clinical documentation.
Collaborates within the quality team to design innovative patient outreach strategies to close gaps in care.
Other duties as assigned.
QUALIFICATIONS:
Exceptional verbal and written communication skills are required.
Ability to learn and efficiently utilize electronic health records (EHRs).
EPIC experience preferred.
Two or more years of experience working in healthcare; primary care experience preferred.
LPN or LVN required.
